This form is for employers and benefits brokers interested in learning more about Centivo’s self-funded health plans. If you’re a current Centivo member or a provider, please see the routing information below.

• I'm a Centivo member - where should I go for support?

Member support is available through the number on your Centivo ID card, or through your member portal at centivo.com/for-members/. This contact form is not monitored for member inquiries.

• I'm a provider looking for eligibility verification or claims support - where should I go?

All provider inquiries are handled through the Centivo provider portal. You can verify eligibility, submit claims, and reach provider support there.

• What should I include in my message?

Include your company name, approximate employee count, current health plan setup, and the best way to reach you. If you’re a broker, let us know which markets you serve and we’ll connect you with the right person.
